The machine is the smaller half of the bill over time. Masks, cushions, tubing and filters are replaced on a schedule, and that schedule is what most of the long-run cost is made of.

This is a ceiling, not a recommendation. It is the most Medicare will pay for, and parts are replaced when they need replacing rather than because the calendar allows it. Take the cushion line as the example: two a month at the manufacturer's $36 each would be $864 a year for one small silicone part, which is a good reason to check what your own supplier actually ships you.

The machine line is per rental month, not per machine. At $47.42 a month your 20% comes to roughly $9.48, and across a full 13-month rental that is on the order of $120, plus the annual deductible if you have not met it. Read that total as approximate: the figure in the table is an average across all rental months billed in the year, and Medicare pays a reduced amount in the later months of a capped rental.
The cushion row is worth a second look. Code A7032 was billed 7,519,268 times in the year, more than any other item here, and code A7038 for disposable filters was billed 23,201,087 times. The small parts are where the volume is.
The rules that set your share

The 12-week trial matters financially as well as clinically. If the follow-up visit does not document that the therapy is helping, coverage does not continue, and the rental stops there.
Buying outright compared with renting
Paying cash gets you the machine on day one, with no adherence reporting and no supplier in the middle. It costs about $1,004 up front and you buy every replacement part yourself at list price.
Renting through Medicare spreads the cost, includes the humidifier and the supplies on the same 20% terms, and hands you the machine after 13 months. It requires a diagnosis that meets the coverage criteria, an in-person follow-up visit, and continued use that the supplier reports.
Between them sits the case that catches people out. If you already own a machine from before you joined Medicare, coverage of replacement parts is possible but conditional, and it needs documentation of a qualifying sleep test from before your enrolment. Ask about that before you assume the supplies are covered.
When to raise it with a clinician instead
Cost is not the first question in some situations.
- You have stopped using the machine because it is uncomfortable, and daytime sleepiness has come back
- The pressure feels wrong, or you wake with a dry throat or stomach bloating most mornings
- You have nearly fallen asleep while driving
- Your mask leaks every night and no cushion size has helped
A mask that does not fit is a common reason therapy is abandoned, and it is usually solved by refitting rather than by buying a different machine.
